Waivers

All students must complete the Senior Release Form, and any student who has a life-threatening allergy or condition must have an Emergency Care Plan on file to attend Grad Night. Please return these waivers as soon as possible to chspo.chair@gmail.com. All forms must be completed and returned to CHSPO by June 1, 2024.

We strongly recommend having a parent/guardian sign all waivers so they are aware of the risks of the party and assume responsibility for paying for any damages their senior may cause at the party, but if the senior is already 18, the parent/guardian signature is not legally required.

Senior Release Waiver: This is the general party agreement that each senior and their parent/guardian must sign to attend the party. This is essentially a permission form that students would fill out for school trips.

Emergency Care PlanNot all seniors will need this form. This waiver must be filled out by seniors with any life-threatening allergy or condition to ensure we have the proper care information in an emergency.
